12 auto revs than slams into gear. Anyone?

Default 12 auto revs than slams into gear. Anyone?
For the last couple of short drives I'm going up a hill and the trans revs and slams into gear. I can be in fourth or third when it happens.

10k miles on it and trans fluid is level. Any ideas? Thank you
