If misisng or FUBAR'd, get it from an online wrecking yard.
If cosmetically damaged, then order the padded covers from 4wd.com. These covers come in black and add 1/2" of padding to the annoying rock hard headrests. I love mine! But they also will cover up cosmetic damage.
The only cost about $30 a pair...
