Great stuff! I'm glad no one was hurt when you experienced the failure since I'm sure that had to be a pretty scary situation.

This time around, make sure to use red loc-tite and make sure those Jam nuts are nice and tight. I believe most recommend 200+ft-lbs of torque on those jam nuts. Basically, put your big boy pants on and grab the longest handle wrench you can find and tighten them as hard as you can. Then put a paint pen across the nut and the arm for simple visual inspection to check if the nut is loosening at all over time.
