1. You don't take off the cap. You may, in fact, have broken the little tabs that hold it onto the knob. A friend of mine did that to mine and I have successfully hot glued it back together.
2. The knob is most easily taken off by tapping up from the bottom with a hammer. Wrap strong tape just below the knob around the shaft. Packaging tape works well. This is to protect the shaft in step 3 and 4.
3. Place an adjustable wrench, slightly wider than the diameter of the shaft, just below the bottom of the knob on the shaft.
4. Tap lightly up with a hammer, driving the knob off. If you tap too hard, it will come off quickly and bop you in the nose. Eye. Face. You get the idea. If you're uncertain how hard to tap, then place a towel over the knob to catch it as it comes off--just as you would a Champagne bottle cork.
5. To put it back on, just press down.

Mine was on so tight that "lightly tapping" was a no go. It took some serious hits from a light sledge and an open end wrench held in a pipe to get it off.
Keep in mind that while everything is standardized as much as possible, there are still variations in manufacturing. You get an unusually wide tcase lever and an unusually tight handle on the same vehicle and it is going to create some problems.
Good luck and hope yours isn't as tight as mine was.

1. Pop off the shifter cap with a small flat-bladed screwdriver.
2. Using the same screwdriver, pop off the metal retaining ring (or use a scribe).
3. Slip the knob off of the xfer case shaft.
NOTE: I put my xfer case in Neutral first, for easier access. Make sure you use the parking brake!
