get a 6LN (6 inch long nose) set of vise-grip pliers. A NEW SET, around 10 bucks. Hold the nutsert around the head , get an impact wrench and zip the bolt out. An impact will ''break'' the bolts' hold on the nutsert...
A new set has sharp teeth that will grip without crushing the nutsert head....
