Default More Carnage

Well I'm definitely paying for having a heavy foot. 2 months ago I blew my clutch out on the trail in mud (I thought it was just the clutch anyways). I fixed the clutch and bolted up the front drive shaft and found that the front diff was also blown. Finally fixed that last weekend and finally got the jeep back on the road. Well last night driving down the road I hear more carnage. This is what I found, my rear diff also blown :(. Definitely driver error right? Not a manufacturer's defect? Anyone see anything like this before? Everyone of the ring bolts were ripped out.

Do you guys think the bolts could just rip out like that? Or do you think it could be an installation error? I could see one or two backing out like that if they weren't tightened properly during install but I wouldn't think they all would back out like that, makes me think they just ripped out. But I have no idea.
Not to mention I just blew the front (Spider gears lost all their teeth) so also makes me think this rear damage was due to the same incident and not installation error.
I would appreciate your opinion though.

Ouch... That looks like a installation issue to me, Either not torqued properly and/or no thread locker installed on the bolts when the ring gear was installed.
