Look, we just balance it - it takes a lot of practice, but, after a while, you get good at it.

I've not seen one that was not part of a aftermarket rear bumper or swing away tire. The plastic bumper just will not hold up to the weight, plus the very limited space available.

I just recently saw a TJ with a warn rear bumper. It looked like there was a 4" x 4"piece of angle iron either bolted or welded to the bumper outside where the spare tire is then the hi lift was bolted and locked to it. Looked pretty simple and clean and the jack was upright as you explained. I'm sure if you found a good welding shop they could fab something for you. This looked good and I wish there was a way to do it with the stock plastic one.
