I include the spare because, without a full-blown lift, it's a lot easier and safer to rotate the tires when there's an extra one at hand (jack one corner at a time).

Default 5 Tire Rotation

Count me as another one for the five tire rotation. I had the local dealer do it, because I was feeling lazy, and going on a trip this weekend. I had to argue with them to get them to include the spare... like its the first time they ever did it. I finally pulled out the owners manual "tire pamphlets" that come with the new vehicles, and pointed to it and said "do it this way!" before they agreed.

Another vote for 5 tire rotation.
I had tire place do it for me.
They include all tire rotations/flat repair in the price of the tires.
Small local guy who gives city cops and firefighters a little break on price too.
I asked them to include the spare and all they said was "OK".
