Have you verified the temp sensor is reading correctly?
Get an infrared temp gun and point it between the thermostat housing and the temp sensor.
It should read between 195°F and 215°F, with the needle straight up. If the needle is to the right in this temp range, the sensor is bad.
The fan should come on around 215°F.

So I finally figured it out, after I ordered a new radiator. Turns out my old one was still clogged, cuz I couldn't get between the AC cooler and radiator, note to self tear it a apart first then by the parts. Thank everyone for the tips/ help.
