Dumb question: Teraflex 2.5 BB and wheels spacers

I think a good question at this point would be:
Has ANYONE had a spacer, steel or Aluminum, fail?
Considering that people find ways to break rims and hubs, etc...it shold not be impossible to find a few broken spacers...but, I have not seen one yet.
But then again, I tend to wheel with people who tend to just get rims with less BS for $45/rim, than buy spacers at $50/rim, etc.
