Does that mean you use this new bracket and remove the old bracket from the passenger side?

YES once you have the new Rear Track Bracket you no longer need the shock relocation mount for the passenger side.

Yes, I was told to throw the old frame mounted bracket away. I am putting on my new one tomorrow.

Yes then DEFINITELY check the welds on the factory bracket to see if any of the paint has popped off of them. This is a positive sign the weld is cracked and weak.
If the paint is still intact your good to go!

Oh and mount the track bar to the top hole of the bracket (there are 3). As in the picture I posted above.

If you call Teraflex they will ship you one free! IF you purchased the 3" kit.

Like whpony96 said, they will send you one if you have the 3" kit. If you just want to buy one, you will have to get it from one of their distributors. The part number is posted in the billboard area on here, if it wasn't already posted in this thread.
