I hope the doctors are wrong and its not terminal but if it is, either way, the most valuable thing Ken and his family can have right now is time. Ok, so i threw this together but maybe this will get the creativity flowing. A simple mono tone design makes for vinyl stickers. This is a 'scratch' design.

With the 'surly' character in mind, it made me think of Treasure Island and with my love of all things pirate, you get this.
