Unable to get driver's side mirror off
if it's any help, I was able to remove the mirror off my pickup by backing it into the side of the garage door.
worked really great, the mirror was off in a fraction of a second.
also stripped the paint off the outside of the garage nicely.

* Update, I still don't have it off. I've tried everything in this thread a dozen times. I keep spraying rust eater on it, hoping eventually something will give. I even had it at the dealership the other day for an oil change and made a game out of who could get it off. About 5 service techs tried with no luck as well.
Take out one of the bolts that is holding the passenger side mirror to the door. It's the same thread as the one you removed from the bottom of the mirror, except it's longer. Thread that all the way in and then give it a few sharp smacks from a hammer. It will pop the mirror up. I just did this on Saturday with both of mine.
Okay....I just went through this very same problem last week. Rest assured that your mirrors will eventually come out. Forget what the instructions say about a slight tap/force. I had to use a hammer and bang on the top portion of the factory bracket with a hammer while I was holding the mirror. I would use a rubber mallet or something to protect the OEM bracket if I were you. The point is that you are going to have to get a little forceful if you are ever going to get those mirrors off.
"some force" is an understatement. I broke the houseing around the driver's side mirror before I was able to remove it. I used the advise above and threaded the bolt in a few turns. After several hits with a hammer the mirror popped out. Passenger side was much easier to remove.
